New Patient Information
We recommend that all new patients start with a full mouth series of diagnostic X-rays, then have a cleaning, and an examination. After that, we can discuss your dental condition with you, and the treatment that you may be in need of.
If you have X-rays from your previous dentist, it would be helpful for us to have those. Insurance will only pay for a full mouth series of diagnostic X-rays once every three to five years. We cannot do a proper examination without a full mouth series of diagnostic X-rays that is less than 3 years old. Bitewing x-rays of the molars are often taken every year.
When you come in to our office, we will need you to fill out a registration form and health history. This takes usually about 5 or 10 minutes. If you have dental insurance, we prefer to make a copy of your card, to get all of the information about the plan. You will also need a photo ID.
